Our GP Referral Scheme has now been running since 2003 and has successfully helped well over 150 clients to an habitually more active lifestyle.
Exercise on Referral
The concept of exercise on referral is to provide a partnership where medical professionals can indentify suitable clients to be referred to fitness facilities for safe and appropriate physical activity (PA) interventions.
Aims
The Lincolnshire YMCA GP Referral Scheme will encourage and help participation in a variety of PA in conjunction with and complementing the role of primary health care. It endeavours to educate all clients in the holistic benefits of regular PA and in doing so lead the client to an independently more physical lifestyle.
How, how, how!
To access the scheme you must first be referred by your GP or other health professional such as practice nurse or health visitor. The medical professional will assess your suitability to join the scheme and, if you are successful, then a referral form will be completed and sent to us here at the Centre. Your details will be placed on a short waiting list and you will be invited, as soon as possible, down to the Centre for your welcome session.
- The programme will run for twelve week with the amount of sessions varying depending on the ability of the individual client. During the twelve weeks all activities undertaken within the Centre will be provided at a greatly reduced rate .... please enquire for further details.
- During the programme each client is carefully monitored according to their needs and ulitmately encouraged to live a more self reliant and active lifestyle.
Client Categories
- Category A: No medical conditions (low risk), physical activity for apparently healthy individuals with no physical limitations and not more than one CHD risk factor.
- Category B: Simple medical conditions (medium risk), adapted physical activity for people with minor stable limitations or two or less CHD risk factors.
